I am interested in this, so if anyone knows to answer the below questions I would appreciate it!

Does this normally have a dust jacket or a sleeve?

I believe there were two versions of this book, was the second one a softcover one?

It did not come with a dust-jacket or sleeve, the first edition was hardback and the latest edition is softcover

Looks okay to me, but would like to see what the actual provenance is instead of just a description of what it is from the auction site. Also would like to see what Aelfwine thinks about it too.

The handwriting of the inscription and the signature are both correct. (I'll leave it to others to speculate on why the ink of the inscription differs in color from that of the signature.)
