Ed wanted to do something special for the fourth anniversary issue, so he asked Diana Paxton (a regular contributor and friend) to do a portfolio of six full page illustrations. The portfolio was laid in to the issue (not stapled or attached in any way) and it was forgotten to be mentioned in the table of contents or anywhere else in the issue.
